The Illinois Continuity of Care Association convenes more than 450 professionals committed to high quality post acute care by fulfilling its purpose:
- To promote high standards of health care
- To promote the interrelationship between all allied health professions in discharge planning activities
- To develop and promote public awareness of continuity of care
- To provide resources and education programs to enhance the delivery of continuity of care
