MD for Seniors

About MD For Seniors

MD for Seniors is a visiting-physician practice founded by Dr. Kojo Pobee in 2006, serving seniors across the Dallas–Fort Worth Metroplex. The team brings primary care directly to patients in nursing homes, assisted-living communities, and group homes across dozens of facilities.

Care is delivered by a mobile clinical team of five providers (two physicians and three nurse practitioners) who round at different facilities throughout the day, rarely in the same building at the same time. Day to day, the practice coordinates most heavily with facility staff, and also with hospice agencies, home-health agencies, and durable-medical-equipment companies, all while staying in sync internally.

Situation at a Glance

Md for Seniors model depends on tight, real-time communication: providers are constantly on the move between facilities, fielding calls from the sites they are not physically at, signing orders and labs, handling refills, and making fast decisions about whether a patient needs to be sent out to the hospital. Because this is healthcare, every one of those exchanges has to be HIPAA-compliant, ordinary texting is not an option.

The practice had adopted an earlier communication app plus a phone answering service, but those systems could not keep up with how the team actually worked. Simple things took too long, conversations were capped and fragmented, and there was no patient-level organization to fall back on when a new staff member entered a case mid-stream.

Challenges

1

One-way communication

  • On the previous app, providers could only receive and reply, they could not initiate a message to a facility. If a provider left a building and remembered something, there was no way to reach out; they had to wait to be contacted.

2

No mobile access for facilities, no patient channels

  • The old app was mobile for providers but desktop/laptop-only for facility staff. With no individual patient channels, it was hard to follow a case, especially when the nurse now calling hadn’t been involved earlier, so providers often couldn’t tell where a given case stood.

3

Capped, cumbersome conversations

  • The prior system limited each encounter to three back-and-forth messages before forcing a re-login, and sharing a photo was awkward. Despite repeated requests over a couple of years, the vendor was unresponsive to adding mobile or other improvements.

4

HIPAA exposure

  • The team needed the simplicity of texting but couldn’t use it; patient information demands HIPAA compliance, and insecure communication created real regulatory risk for the practice and the facilities it serves.

It saves time, and you can impact patient care in a more comprehensive manner.

Solution: Hucu’s Patient-Specific Communication

When MD for Seniors evaluated a replacement, the must-haves were clear: true mobile access, the ability to initiate two-way conversations, and effortless in-context photo sharing  all without compromising HIPAA. Hucu delivered on each.

Two-Way, Provider-Initiated Messaging

Providers can now start a conversation the moment they need to, even after leaving a facility, instead of waiting to be contacted. Threads continue until the issue is fully resolved, with none of the three-message cap that forced repeated logins on the old system. Exchanges are longer but more complete, giving a fuller picture.

Patient-Specific Channels

Everything about a patient lives in one channel. When facility staff change, which happens often; incoming staff don’t have to start over: Dr. Pobee can simply point them to a response already in the channel rather than retyping it. It is the feature he calls one of the biggest things about Hucu.

Mobile-First with Easy, Flexible Login

Hucu works on any device, with multiple low-friction sign-in options (Dr. Pobee specifically likes the PIN method). That flexibility matters across a network of facilities where staff comfort with mobile varies.

Effortless In-Context Photos

A quick photo of a skin lesion, an eye, or a medication for a refill goes straight into the patient channel turning guesswork into an accurate, confident diagnosis and the right treatment, faster.

Full HIPAA Compliance: Protecting Providers and Facilities

Every message is secure, recorded, and defensible, making it easy to prove an order or refill was handled. Dr. Pobee notes facilities often don’t realize their other communication channels expose them to liability; with Hucu, MD for Seniors removes that risk from the relationship.

Responsive Partnership & Support

Unlike the prior vendor that ignored years of feature requests, Hucu’s team actively supports onboarding - the practice’s main remaining task is simply signing up new facility staff as they rotate in.

Real World Success

The clearest example of impact is also the simplest: the photo. When a facility nurse tries to describe a skin lesion or an eye problem over the phone, the possibilities are nearly endless and a confident diagnosis is impossible. With Hucu, a single picture drops into the patient’s channel and Dr. Pobee can immediately identify the problem and order the right treatment, rather than prescribing a temporary fix until the next in-person visit. Care is delivered faster, and the right care is delivered with confidence. He describes the same effect on send-out decisions: with a complete picture in hand, the team can decide more quickly and comprehensively whether a patient needs to go to the hospital.

“Communication is easy. We hardly ever have communication problems, because our method of communication is Hucu.”
